5 days getting sensitivity to cold & hot on upper left molar.

5 days getting sensitivity to cold & hot on upper left molar. Got great comments from 1st post. Went to Dentist. X-Ray no crack, decay, or infection. Examined tooth & deep filling but okay. Did bite check: ok. Sprayed air & was little sensitive but went away quick. Use Endo Ice & didn't feel anything (little short sensitivity) She gave me prevident, motrin, & told me I might be grinding my teeth, use mouth guard. Don't have constant pain/ irritation on gum. RCT or just grinding teeth?

You went to a very thorough dentist. Give it time and stick with her!
The only desensitizing toothpaste I feels works is on that has 5% Potasium Nitrate, such as Pro relief by Cogate (http://www.colgatesensitiveprorelief.com/EN/home) brush with it and put some on a cue tip and rub it on the exposed root surface for 15 seconds do this each time you brush.
I would recommend you to wait for a few days . Sometimes the nerve could be slowly dying and the alive fibers repond to the vitality test - such as cold and air .

If the pain goes away completely - get the vitally tests done again - prefer the electric pulp tester - if no vitality - rct due to necrotic nerve , if pain worsens - then rct due to irreversible pulpitis.

But If pain goes away and tooth is still vital - then no tx

And yes Do wear the Nightguard as recommended by your dentist !! You are in good hands

90% of the time this is a grinding clenching issue if the 4 cardinal signs of tooth pulpitis are not present. 1. sensative to percusion. 2. sensative to cold that lingers. 3 radiological evidence of non vitality 4 severe dull toothache. checking the bite for occlusal interferences and making the adjustments. Because the filling is deep it still may become an endodontic issue.

Give it some time ! Once you cut on a tooth you can't uncut a tooth, you may have some recession leading too root sensitivity. Try desensitizing agents like fluoride varnish first!
Xray will not determine crack. Sensitivity to hot bad sign usually indicates root canal looms. Could be a fractured tooth. If vertical fracture end of the line for the tooth

Sounds like your dentist gave you good advice. Only time will tell. It may be cracked. Often the cracks are not detectable. I am concerned about heat sensitivity.

It could be several different things: grinding, gum recession, internal crack caused by deep filling, or recurrent decay blocked by the filling on the x-ray. Did she check for cracks with either a Tooth Slooth or Frac Finder? I have found if you have a large, deep amalgam filling, that over time, if you are a grinder and internal crack will develop. Then, when you grind your teeth, the cusp will wedge apart, causing the nerve endings to "stretch". This would lead to a reversible pulpitis and if not treated, an irreversible pulpitis. If it progressed to an irreversible pulpitis, a root canal would be needed.

Another thing with molars is that there are multiple roots, therefore multiple nerves. Usually, a molar will have 3 nerves (2, 4 or even 5 nerves in a molar are possibilities). One nerve will be a large nerve and the other 2 are small nerves, easy to damage. So you can have nerve damage or death of the small nerves, but have the big nerve (usually the palatal root nerve) be healthy and give a false positive that the tooth is healthy. So, after Prevident use and a nightguard, the symptoms persist, go back to the dentist for a re-evaluation. Stick with this one as it seems that she knows what she is doing.!! Good luck!!!!!!!!!!!!!!

I believe this could definitely be from grinding your teeth. I see this weekly and think that you should consider having a bite adjustment and then also wear a night guard. If it doesn't get better then possibly you will need a root canal. Good luck

"Hot" sensitivity frequently indicates an impending nerve problem. Bad bites, deep fillings etc. often aggravate the situation. Is your tooth a "time bomb"?, maybe. My gut feeling is that you'll need RCT at some point. When, is another question. All the things that you're doing may be all well and good in the short term, but it's difficult to tell exactly what the future holds for this tooth. See an OFP dentist or root canal specialist for another opinion. Then decide.
