Why are you using anabolic steroid cream and have you been applying it to this lesion?
Looks like a cold sore, may try zovirax cream. If doesn't go away in few weeks, see a dermatologist for a biopsy.
It looks like a classic herpes simplex outbreak. And it could spread to the upper lip. Be sure not to kiss anyone or engage in oral sex until it is completely resolved. You could get a viral culture done but they are not 100% diagnostic

unfortunately when it breaks out like this there is nothing that will get rid of it. It should go away in a week or so, and your doctor can prescribe valtrex or another anti herpes drug to prevent outbreaks in the future.
The fact you've had it only a few days makes cancer and other things less likely. I agree with a possible bacterial infection and the antibiotic cream your doctor put you on. If this doesn't resolve it, see your doctor again and an oral antibiotic or dermatology referral might be necessary. If it persists despite this, I agree with the others who recommend biopsy.
4 day history goes against a tumor--think possible skin infection of bacterial type [ecthyma] triggered by a possble cold sore or a scratch/pimple. Return to you Doctor for an antibiotic. If the lesion persists, a biopsy may be necessary to rule out tumor here.

With it having occurred just over the past 4 days, infection is likely. However, the rolled edges give it an appearance of a basal cell carcinoma. You should see a dermatologist for further evaluation.
I'd recommend you go see a dermatologist ASAP.
