Hi there, Please my girlfriend had a medical abortion using pills prescribed by a medical doctor yesterday. She started bleeding and having cramps 2hrs after taking the pills. At about 3hrs later, she went back to see the doctor as the bleed was much; the doctor used some means to stop the bleeding after like 1hr later. Today, the ultrasound scan results says she's still pregnant. My question are how realiable is an ultrasound scan 1 day after a medical abortion and what else can we do now. Thanks.

I am glad that the second ultrasound confirmed everything okay. Now, what I don't understand is why you are considering surgical abortion of a 9 week fetus when you guys plan to get married in a few years. Even in the best hands instrumental evacuation of the womb is quite risky.
Bleeding can be a problem, especially if there is placenta acreta (tightly adherent to the womb). The womb itself has been softened by progesterone at this point, and is very vulnerable to rupture with surgical instruments. Infection in a bloody womb can easily become systemic, and even lead to disseminated intravascular coagulopathy, which lethal.
Wound infection poses the risk of endometrial (inner womb surface) scarring and fallopian tube obstruction. The latter two will in turn pose infertility problem when you get married and feel ready for babies. You may need counseling before you make up your mind on where to go from here.

It would appear that this is not an early pregnancy. Hence the Coyotes did not work. But the important thing is that your doctor has controlled the bleeding. If the ultrasound says the pregnancy is still there then you have to let your doctor decide what next to to do. If the fetal heart sound can be heard, then the pregnancy might still be viable. Other wise you guys have to wait until your girl friend's womb spontaneously ejects the non-violent fetus. When this happens, she must be seen by your doctor to control the bleeding. I wish her a safe outcome.
I would discuss the ultrasound results with your doctor. They may mean that she still has not passed everything as of yet. Cytotec alone will cause an abortion about 90% of the time. If it does not work, you can try again in 3 days.
If there was heart activity on the ultrasound, the probability is that the pills did not work and if she still wants to abort, she may need a surgical abortion. If there was no heart activity, it may be that she just has not passed the tissue yet, or that the pregnancy is very early, or both.. She needs to be followed longitudinally to be sure of exactly what is happening. You did not mention how far along she was, and that would make a difference as to what showed up on USG as well.
