Rash on two year old and large bump.

My two year old daughter has a rash between her legs hat my pretty sure is a heart rash, but on the back of her left she has a bump about nickel size thats red and is kind of itching. She had a fever, headache, and threw up two nights ago.

An itchy bump is unlikely to be a lymph node. A lymph node will enlarge but it will be subcutaneous (under the skin) and thus not itch; it may be painful, but will not itch.

I would be be suspicious that she had a bug bite, or developed a break in her skin due to scratching, and that area has become secondarily infected. It is concerning that she is having signs that there is a systemic infection such as fever. If the area is spreading or getting worse, she will undoubtedly need antibiotics. If there is an abscess developing (pus underneath) that will likely need to be drained either in the pediatrician's office (if he or she feels comfortable) or in the emergency department. If MRSA is prevalent in your area--ask your doctor--antibiotics will have to be chosen that cover for this (trimethoprim-sulfamethoxazole or clindamycin, for example) or she will need close followup if a non-MRSA active drug is chosen, which is also a reasonable choice if the area does not look too bad.
