I have a very bothersome genital problem

Sounds like a yeast infection. Might try some antifungal cream.
Consider seeing a urologist, treat the infection, and getting the foreskin removed as you may have recurrent infections through out your life. This is why it is often done after birth.
Fungal infection most likely. If u are diabetic then u r more prone. U need antifungal creme and get your blood sugar checked, posdibly hba1c
