Try this list for starters: trauma, bacteria in decay, bacteria in gum disease, fracture of the tooth, fractured filling, leaking crown or filling, root exposure caused by the recession off the gums, gum surgery to remove pockets, plaque (think "trench mouth"), clenching/grinding, deep filling, deep cavity, improperly fitted (too tight) crown or filling, improper bite adjustment of a crown or filling, aggressive dental hygiene cleaning, root canal infection, herpetic viral infection, extreme cold (think "brain-freeze"), sinus infection, allergic rhinitis, chemical burn, thermal burn, bone infection, bone fracture, galvanic/electrical reactions, impacted tooth, food impaction between teeth, food impaction under the gums, braces, ill-fitting dentures, ill-fitting retainers, ill-fitting night guard, concussion, congenital disorders of enamel and/or dentin formation, cancer of the brain, spinal cord or nerves of the face and/or jaw, post-herpetic neuritis, and "idiopathic" problems.
