Sound like he might have Tinea Pedis (Athlete's Foot), which is a fungal infection. Try over-the-counter topical creams 2-3 times per day, and see if the problem clears. If not, take him to a Podiatrist in yoru area
Tinea pedis will be the most common diagnosis, but it can be other things too.
Try clotrimazole cream for a fungal infection. These are most likely for young males.
Without seeing the picture it is difficult to say for sure but in my practice I have seen the following happen numerous times. A patient develops a fungal infection which generally causes itching. A patient scratches the area inflaming it and the skin subsequently breaks down resulting in an open wound which then can easily become infected causing the area to swell.
As mentioned by the other doctors, I was not able to see a picture so it is hard to say how severe the rash is. Regardless, if open sores are present, this could be a bacterial infection of the skin and could be serious. This may be either a pronounced case of Athlete's Foot (fungal infection of the skin of the feet) or chronic inflammatory skin condition (e.g. eczema, atopic dermatitis, a contact allergy / dermatitis) or several other more rare skin conditions. My best advice is to have it evaluated by podiatrist in your area to obtain the correct diagnosis and treatment plan.

I did not get a picture of anything. With a skin condition such as this, especially with swelling and severe itching, the best bet is to get a BIOPSY. This is a very quick in-office procedure that any Podiatrist can perform. Once the tissue is examined by a dermatopathologist (a physician who examines skin and soft tissue scrapings and biopsies) a definite diagnosis can be made and the proper treatment started.

This may be a condition called dishydrotic eczema and can lead to bacterial infection. This should be evaluated by a podiatrist ASAP.
