- This competition is open to residents of the USA and Canada.

- Each Qualifying Review constitutes one entry into the competition. There is no limit to the number of entries which any one person can make, however, only one review per office visit is allowed.

Any review classified as not authentic, offensive, or inaccurate, or potentially libelous may be removed by DoctorBase editors and such reviews will be ineligible for the competition.
