Sounds like you have a forward head weight -bearing posture which is usually the result of a reversed neck curve (kyphosis).
An aggressive at-home neck exercise that I prefer (because I have seen many good measured results) is the "Posture Pump" (www.posturepump.com). The exercise takes 10-15 minutes 3-5 days each week and over time you will improve the forward neck position and bring the head back over the shoulders.
Remember though, it took your whole lifetime to get the curve where it is today so be patient, do the exercise and your efforts will pay off.
