We Participate in All Major DENTAL PPO networks ONLY. We also accept all other dental insurances as an out of network provider. Please make sure you provide us with copy of your current DENTAL Insurance, the breakdown of coverage and the claims address where we need to send the claims to. All payments including co-pays, deductibles etc needs to be paid at the time of your visit. An insurance contract is between you and your insurance carrier. We are only a Dental provider and Please NOTE that you are ultimately responsible for all payments and NOT your insurance carrier. So please provide us with all current details to help us to submit your claim in a timely fashion. Also Please note that any claim submitted over 30 days to your insurance carrier will not be accepted by them and you will be responsible for the total cost of treatment.
