It sounds as if he would do well with an explantation and mastopexy. It's not uncommon for somebody who has had implants for 30 years to have the rupture. While this more than likely will not make she systemically ill it a potentially will become harder to remove all the silicone the longer you wait. I would recommend seeing a plastic surgeon in your area to get this taken care of.
