Nothing is mandatory. You don't have to have the procedure at all. If you are going to have it though, "cosmetic" in this case means that your resulting bone is likely to cover more root structure and therefore you are getting greater effect for your time and money and discomfort. Not letting a doctor treat you in the best manner possible is like telling a carpenter to build you a house but saying drywall is too expensive and you'll live with just the framed in walls.
That too would be "cosmetic" but it would have a much less effective result. Same here. Don't ask the police to defend you but say they can't use bullets. This is, in my opinion, not the time to be frugal (cheap).
