There are several factors to consider in this situation. First and foremost, is your diagnosis even correct? I would recommend possibly a second opinion regarding your diagnosis. There are many conditions that can mimic sacroiliac joint pain, similarly, sacroiliac joint pain can mimic many other conditions. If you have had all of the treatments listed above, I am concerned that you might be heading down the wrong path.

On the other hand, there are newer surgical techniques to address sacroiliac joint pain refractory to typical treatments, yet this should only be recommended if you have had some positive response to prior diagnostic injections. In this case I do not see that this has occurred.

As others have mentioned, further workup by a pain specialist/physiatrist is recommended, as there may still be nonsurgical strategies that can help you with your pain condition.
