Many doctors will leave old fillings when they place crowns. Over the years I have noticed most of the old silver fillings have decay under them. My practice is to remove the filling as part of the preparation for the crown. I place a new filling if there is too much tooth structure missing once the filling and decay is removed, otherwise the new crown may replace the missing structure without a new filling.
