This could be a few different things, without having you in person and getting more history it is not possible to say exactly what is going on. It could be as simple as your Levator Scapulae muscle being hyper-tonic, which can occur from spinal issues in the neck. You should seek a chiropractor or Osteopath that will look at it from a musculoskeletal view and do some soft tissue and spinal work with you, rather than just getting painkillers and muscle relaxers, though they may be needed as well to help you deal with the pain.
