Your teeth are part of the problem. Broken teeth especially breakage into the nerve can be causing root canal problems/infection. If your roots extend into your sinus cavity it can cause sinus problems especially if multiple teeth are involved. Because of some of the medications you are on it is masking this pain/problem.
Also since many of the teeth are broken your jaw joint is not being properly supported by your teeth thus resulting in a possible TMJ or jaw joint pain which you may be misinterpreting as ear aches.
Thrush is usually brought about by being on long term antibiotic therapy. Diflucan 150 mg taken once should help this situation.
As far as your overall problem, you need to have xrays to evaluate all your teeth and evaluate which ones can be saved and which ones need to be extracted. See if the oral surgeon works with a understanding restorative dentist who can help with the broken teeth or possibly a hospital based dental program or dental school for your dental evaluation and care.
