This is a bit of a loaded question in that you will probably find as many answers as there are doctors. I do not perform rhinoplasty, but for other cosmetic procedures, I have a policy that any revision surgery must at least be addressed within one year from the date of the original surgery. Now, whether or not that revision is "covered" is a separate question and will depend on both the doctor and the type of surgery or circumstances. For example, if I have done liposuction on someone and they come in 6 months later, 5 lbs heavier, and tell me they don't see a difference and want me to re-do the lipo at no cost to them, we have a problem. Likewise, with procedures that have a high revision rate, that should be part of the pre-operative counseling and you are smart to be thinking about it now. Many doctors will include the cost of potential revisions in the price of the original surgery and many do not. I encourage you to do as you are doing: get some facts and ask your doctor about the revision rate and if there is additional cost to you if you were to need a revision.
