Thank you for your question. Over time, implants of any kind, including breast implants can fail. Failure of Saline filled implants will show a progressive loss of volume over time. Since it has been a year with no changes in your breast size or shape, it is unlikely there is a saline implant leak. However, the new "dent or wrinkle" of the breast may indicate problems with breast tissues not directly associated with your breast implants.
You would be best served consulting with a board certified Plastic Surgeon to establish a diagnosis. Physical examination and appropriate imaging studies should be performed to ensure you have no issues with your natural breast tissue. Once a diagnosis is established, appropriate surgical recommendations can be offered.
