Hi. My cousin (she is 17 now) has one foot that is 3-4 sizes smaller than the other foot. She is also kinda short for her age (at 152cm for a 17 year old). She has been to a pediatrician when she was younger and did a xray for her foot but the doctor said that there was no need to be worried unless she is in pain. Considering the fact that her height is short for her age( her mum and dad's height are 160cm and 170cm respectively), is this of a concern?

I would not be concerned either based on the information provided. However, she may be dealing with a metabolic disorder. It is hard to say without actually examining the patient. Long term issues related to gait abnormality may be a greater concern.
I would suggest seeing a foot and ankle specialist in tandem with an adolescent medicine doctor or primary care physician. She may require orthotics as well.
