Possible/primary diagnosis to what am i suffering

my shoulders up to my hands and fingers are severely in pain but not all of the time it is by rhythm. Everytime i cough it felt like my lungs was going to get out of my rib cage and somewhat i observe that my collar bone on the left side is some protruding compared to the opposite side. I had visited a doctor and taken all of the prescription given to me but still there is no effect. Right now I am somewhat taking pain killers to lessen my suffering. Can you help me what could this be? Thank you

You need a diagnosis and not a "treatment." A diagnosis is based upon a careful history, questions based upon your specific symptoms. This is followed by a careful examination (possibly a Neuroloigcal exam) and appropriate tests.

Without more information it is difficult to provide specific recommendation, but this may help. If you are experiencing pain radiating from your shoulders down to your hands--you most likely have some type of nerve compression. When nerves are injured from the neck they radiate into the arm or hand in very specific distributions. However, when symptoms radiate and encompass the entire arm and hand (or along entire outer or underside)--this is often related to thoracic outlet syndrome (TOS). The TOS space is between the neck and shoulder. The entire plexus of nerves that exit from the side of your neck are located between the muscular layers of the neck and shoulder, and between the collar bone and your first rib below. When you cough these muscles pull upward onto the top of the rib cage and can irritate the nerves that lie above them, thus producing radiating pain when you cough. There a number of causes for this, from overuse activities which tighten the muscles around the TOS to more serious conditions like lung cancer. Successful and effective treatment requires help from an experienced chiropractor (especially if they have Active Release Technique certification), chiropractic neurologist, or physical therapist. If you are experiencing any swelling or discoloration in the arm or noticeable atrophy/ loss of muscle mass in your arms or hands--you must receive some additional testing to rule out risk of developing a blood clot or stroke. Sometimes TOS can compress an artery or vein and pose a risk for this. An ultrasound or MRI/MRA/MRV of the TOS can rule screen for this. Here is a link for more info: http://www.tosmri.com. This is important because you don't want someone poking around in there if this is a high risk for you. Although, not common, it does happen. Most TOS cases recovery just fine with conservative therapy with chiropractic adjustments to improve the mobility of the spine and rib cage, soft tissue therapy to relieve the cumulative tension in the muscles encompassing the thoracic outlet space, nerve gliding techniques and rehab to strengthen and maintain improve posture.
